Support Services Case Manager

The Support Services Case Manager position which will report directly to the Assistant Director in the administration of BIAMD’s Maryland TBI Waiver Contract, The Maryland TBI Advisory Board, BIAMD 's Support Services, and BIAMD’s Member services and outreach.

The Core Responsibilities for this position are:

Conduct intakes with new clients/family members/providers seeking information and referrals as a walk-in, on the BIAMD Maryland Brain Injury Resource Helpline or through email. These intakes include assessing service needs and specific challenges to accessing services. Upon thorough assessment, provide relevant resources, concentrating on assisting in de-institutionalization or diversion from institutionalization.

Develop, execute, and oversee case management services for participants in the Maryland TBI Waiver as assigned, including but not limited to, quarterly visits, quarterly reporting and annual plans of care.

Provide follow-up with clients/family members/providers as appropriate to ensure their receipt of medically appropriate services. Appropriate follow-up may include such things as participating in family meetings, support groups, conference calls or other meetings.

Develop relationships with clients' other care providers, such as case managers, social workers, medical providers, or home health agencies, particularly CARF accredited long-term care facilities, for purposes of coordinating services, assuring continuity of care, and ensuring compliance with plans of service.

Make contact and develop relationships with community organizations serving persons in need of BIAMD services, particularly those long-term care facilities in Maryland which are Commission on Accreditation of Rehabilitation Facilities (CARF) accredited. Distribute written materials as needed.

Maintain, enter, and update data in BIAMD 's electronic database and create paper files where necessary. Provide information and reports as needed.

Attend and participate in BIAMD team meetings for program management and development

Assist with and attend all BIAMD events including, but not limited to, conferences, meetings, and workshops.

Create, update, and distribute brain injury outreach information to the general public using varied social media tools, applications, and outlets.

Perform other duties of position as assigned

Community Outreach & Engagement

Represent BIAMD at community events, health fairs, conferences, and stakeholder meetings.

Build and maintain relationships with service providers, hospitals, rehabilitation centers, case managers, community organizations, and advocacy groups.

Assist with outreach campaigns targeting underserved or high-risk communities across Maryland.

Assist with developing outreach materials, including flyers, email content, social media posts, and community updates.

Maintain BIAMD’s presence on social media platforms by creating engaging and relevant content.

Update resource lists, contact databases, and community partner directories.

Experience/Education/Skills Required

The ideal candidate:

  1. Has earned a Master's degree in Social Work or social service field preferred, or Bachelor's degree in social service field with a minimum of two years of experience in case management or resource coordination

  2. Has five or more years of experience in a direct social service or medical setting. Strong preference for individuals with experience in the field of brain injury (CBIS preferred), the Supplemental Security Income/Social Security Disability Insurance application process, and the SSI/SSDI Outreach, Access, and Recovery (SOAR) initiative .

  3. Has demonstrated ability in engaging clientele or consumers from a wide diversity of communities and backgrounds.

  4. Has an up to date understanding about the progression and treatment of acquired brain injury, treatment options, and resources available to brain injured individuals and their families in Maryland and the surrounding area

  5. Is able to organize and prioritize multiple projects and meet deadlines in a time-sensitive environment.

  6. Has experience creating and implementing programs and initiatives designed to increase membership and community engagement.

  7. Possesses an understanding of strong professional and ethical boundaries.

  8. Has demonstrated skills and working knowledge of Microsoft Office suite of applications including Excel and Word.

  9. Has worked with electronic relational databases.(Preference given to Salesforce and/or LTSS online)

  10. Is able to communicate clearly and professionally in verbal and written form

  11. Is comfortable with public speaking, public education, and public outreach

  12. Has a proven ability to work both independently and as part of a collaborative team.

  13. Is able to develop good working relationships with internal and external clients, members, donors and sponsors.

  14. Has the ability to lift up to 25 lbs, and walk up one flight of stair

  15. Has access to personal transportation.

  16. Is able to work a flexible schedule that will include some evenings and weekends.
