The Harris Center for Mental Health and IDD, known as "The HARRIS CENTER," is seeking an experienced Program Assistant, IDD, TxHmL to join our team. The Texas Home Living Program Assistant serves as the critical link between program compliance, financial oversight, and service delivery for TxHmL and ISS. The TxHmL Program Assistant is responsible for coordinating and monitoring services for individuals enrolled in the Texas Home Living waiver program. This role ensures that individuals receive appropriate supports that promote independence, community integration, and quality of life. The TxHmL Program Assistant serves as a liaison between individuals, families, providers, and regulatory entities, ensuring compliance with state and federal guidelines. The Program Assistant will respond to routine requests and use judgment and initiative (with supervisory consultation) to determine necessary response/action in non-routine situations.

Your role in Action:

-Develops, implements, and monitors Person-Directed Plans (PDP) and Individual Plans of Care (IPC)

-Coordinates services across providers including nursing, respite, CFC, and other TxHmL supports.

-Works collaboratively with Agency stakeholders to fulfill requests and/or share the required information.

-Participates in interdisciplinary team meetings and collaborates with Service Coordinators.

-Supports billing processes by collecting and verifying service documentation.

-Maintain program databases and spreadsheets tracking admissions, discharges, census, and incidents.

-Coordinates duties and maintains accurate documentation in accordance to TxHml Waiver.

-Complies with The Harris Center policies and procedures and contractual requirements including punctuality, attendance and access to the facility.

-Coordinate services offered under the TxHmL waiver program, ensuring participants receive appropriate support that facilitate community living and personal development

-Provides education and support to families regarding available services and rights.

- Assists with revisions to IPCs and service plans as needed.

-Provides transportation for clients as needed or assigned.

-Maintains staff and consumer safety.

-Performs other related duties as assigned.

What Qualifications You'll Need:

Education:

-High School Diploma or General Education diploma

- Bachelor's degree (B.A.) from four-year college or university preferred.

Experience:

-3 years related work experience
-Knowledge of TxHmL waiver services, Medicaid billing, and person-centered planning
-Ability to travel locally and work flexible hours as needed



Knowledge Skills and Abilities:

-Knowledge of Microsoft Word, Outlook, Excel, Access, Power Point and Outlook.
-Knowledge of HIPAA Privacy and Security Rules.
-Possess strong verbal and written communication skills, and interact effectively at all levels within the
organization.
-Possess critical thinking, problems solving and listening skills.
-Must have well-developed interpersonal, organizational and collaborative skills with a desire to provide
a high level of customer service.

-Ability to maintain absolute confidentiality of all work-related matters
