As a Homecare LPN, you will leverage your comprehensive education and experience to deliver personalized care in a supportive environment that champions patient autonomy. Your role will include:

  • Facilitating communication between patients and the interdisciplinary team.
  • Building and nurturing therapeutic relationships while educating patients and encouraging self-advocacy and independence.
  • Utilizing modern technology to accurately document the skilled services you provide.

Qualifications:

  • Valid CT LPN license.
  • Minimum one year experience as an LPN; homecare experience is a plus.
  • Valid driver's license and reliable transportation required.
  • Computer proficiency is essential.
