Since 2016, we’ve made it our mission to keep aging adults healthier, longer—right where they live.Position Summary:We are seeking a motivated Nurse Practitioner (AGNP or CNP) to deliver on-site care to senior living communities. If you’re passionate about serving older adults and want to expand into wound care, this is your chance. We’ll even pay for your wound care certification if you’re not already credentialed.Key Responsibilities:Deliver compassionate primary care in senior living communitiesDevelop and manage care plans, provide wound care, and support chronic disease managementCoordinate with interdisciplinary teams, families, and support staffKeep documentation current and clear in EMR systemsQualifications:Current CNP license in MinnesotaAGNP certification preferredGeriatric or primary care experience preferredReliable transportation and valid auto insuranceStrong communication and clinical judgmentMilitary Occupational Classification (MOC) Codes:Army: 66P – Nurse PractitionerNavy: 1972 – Nurse PractitionerAir Force: 46Y3H – Nurse PractitionerMarines: NP – Nurse PractitionerCoast Guard: HS – Health Services TechnicianJoin Our Team:Ready to elevate your NP career while making a real difference in seniors’ lives?
