Since 2016, weve made it our mission to keep aging adults healthier, longerright where they live.Position Summary:We are seeking a motivated Nurse Practitioner (AGNP or CNP) to deliver on-site care to senior living communities. If youre passionate about serving older adults and want to expand into wound care, this is your chance. Well even pay for your wound care certification if youre not already credentialed.Key Responsibilities: Deliver compassionate primary care in senior living communities Develop and manage care plans, provide wound care, and support chronic disease management Coordinate with interdisciplinary teams, families, and support staff Keep documentation current and clear in EMR systemsQualifications: Current CNP license in Minnesota AGNP certification preferred Geriatric or primary care experience preferred Reliable transportation and valid auto insurance Strong communication and clinical judgmentMilitary Occupational Classification (MOC) Codes:Army: 66P Nurse PractitionerNavy: 1972 Nurse PractitionerAir Force: 46Y3H Nurse PractitionerMarines: NP Nurse PractitionerCoast Guard: HS Health Services TechnicianJoin Our Team:Ready to elevate your NP career while making a real difference in seniors lives?
