We are seeking a dedicated and experienced Wound Care Nurse Practitioner or Physician Assistant to join our innovative medical group. This role offers the unique opportunity to provide 100% mobile wound care, traveling to patients' homes and facilities to deliver high-quality chronic wound care. Initially, the position may require 2-3 days a week with the potential to increase to 4-5 days, depending on patient flow and scheduling.
The ideal candidate will be a compassionate, optimistic team player with a genuine interest in helping others. A strong background in wound management and expertise in chronic wound care is essential. This position requires travel to various locations, where you will provide comprehensive in-home and in-facility wound care services.

Duties:
- Provide advanced wound care treatments in diverse settings including home health, assisted living, and skilled nursing facilities.
- Assess and evaluate wounds to create personalized treatment plans.
- Implement evidence-based wound care protocols and techniques.
- Perform wound debridement, dressing changes, and other specialized wound care procedures.
- Monitor and document the progress of wound healing.
- Educate patients, caregivers, and healthcare staff on effective wound management techniques.
- Collaborate with interdisciplinary team members to optimize patient outcomes.
- Maintain accurate patient records in compliance with regulatory standards.
Qualifications:
- Valid Nurse Practitioner or Physician Assistant licensure in Illinois.
- Must be within commuting distance to our facilities.
- Certification/training and clinical experience in wound care (Wound Care Certification such as WCC, CWCN, CWS or similar, with training available).
- Proficiency in wound assessment, treatment modalities, and wound care products.
Salary: Up to $80/hr depending on experience and certifications.
Schedule: Flexible part-time or full-time options available (Weekdays only, Monday through Friday).
