Case Management Mgr FT JOB RELATIONSHIPS Responsible to the Director of Research and Innovation. Consults with members of the Patient Care Services, Quality and Hospital Leadership staff as appropriate. DEFINITION OF POSITION The Case Management Manager is responsible for providing direct supervision and oversight of the Case Managers and Clinical Social Workers within the inpatient settings. Accountable for the overall day-to-day oversight and management of the case management program including coordination, supervision, and administrative oversight of the case management team. Functions as an expert clinical practitioner, case management subject-matter expert, resource, advisor and leader for the members of the case management team. Monitors quality of the interactions, documentation and assessments of the case management staff. Coordinates activities that promote quality outcomes while supporting a balance of optimal care and appropriate resource utilization. Participates in the review and updating of Case Management policies. Provides education to support regulatory compliance in collaboration with Carteret Health Care Compliance and CM Department leadership. Monitors monthly reporting, including but not limited to, productivity, readmission documentation, avoidable days documentation, denials management and discharge appeals processes. QUALIFICATIONS Professional Two years’ management experience in a healthcare setting required. Two years of post-masters/post-bachelors experience in a health care setting such as a hospital, mental health setting, extended care facility, or home care agency preferred. Knowledge of applicable legal responsibilities/restrictions and current trends in Case Management /social work/social work-related services pertinent to the hospital practice setting. Knowledge of acute, chronic and terminal illnesses, mental and physical disabilities, and the normal developmental process and the impact on family systems. Evidence of participation in ongoing continuing education programs. Knowledge of and adherence to the National Association of Social Workers Code of Ethics. PI6b652493f271-30511-38675760
