The Senior RN Case Manager leads a team of RN Case Managers to ensure quality care in accordance with our agency policies and state regulations. Qualifications Graduate from an accredited nursing school BSN preferred Current, unrestricted RN license in your practicing state Minimum of two years of nursing experience, including at least one year in home care or a related field Preferred one year of supervisory and/or case management experience Current CPR certification Key Responsibilities Fulfill responsibilities laid out in the case manager job description Provide direct care and manage cases for assigned clients Work closely with the regional director to identify and resolve issues and enhance services Plan, implement, and evaluate client care Coordinate and participate in client care conferences as necessary Act as an on-site clinical resource and support for home care needs Collaborate with staffing coordinators for optimal scheduling and staffing Adhere to nursing delegation guidelines and agency policies Assist in interviewing, selecting, and evaluating clinical staff as requested Oversee personnel training, education, and competency validation Review clinical documentation for accuracy and completeness Participate in agency performance improvement initiatives Document and report on infections, occurrences, and performance improvement activities Conduct supervisory visits to facilitate issue resolution Monitor nurse shift reports and ensure adherence to policies Manage home chart completeness and timely staffing Assist with marketing efforts and client outreach as needed Be available during operational hours to participate in all related professional services Perform any other duties and responsibilities deemed necessary The standard pay rate is $44.00 per hour, with additional shift differentials applicable.
