Job Summary: Join the Department of Pediatrics at ETSU as a Practice Transformation Care Coordinator, where you will play a vital role in enhancing the health outcomes of our diverse patient population. This position is designed for a dedicated professional who thrives in a dynamic environment and is passionate about making a difference in the lives of children and their families.

As a Care Coordinator, you will work with chronically ill or high-risk patients ranging from newborns with growth challenges to adolescents facing mental health issues and social complexities. Your expertise in critical thinking and interdisciplinary collaboration will be essential in ensuring effective care coordination across various healthcare settings, ultimately preventing unnecessary emergency visits, hospitalizations, and child protective services involvement.

Daily Activities: A typical day will blend in-person interactions, such as warm handoffs with care teams, and remote communications via phone, secure email, and text. You will document care coordination efforts and participate in team meetings, striving to enhance patient health while reducing overall healthcare costs.

Knowledge, Skills and Abilities
  • Evidence of essential leadership, communication, education, and counseling skills.
  • Proficiency in communication technologies (email, cell phone, etc.).

  • Experience with health IT systems and reports is desirable.

  • Proactively continues to educate self on providing quality care and improving professional skills.
Required Qualifications
  • Master’s degree in Social Work, Psychology, Sociology, Public Health, Nursing, or a related field.
  • One (1) to three (3) years of experience with patient populations receiving medical care or social services.
OR
  • RN with a Certified Case Manager (CCM) credential and one (1) to three (3) years of case management experience.
OR
  • Licensed Practical Nurse (LPN) with five (5) to ten (10) years of relevant experience in case management.
Preferred Qualifications
  • Care coordination and/or case management experience.
  • Bilingual (English and Spanish fluency)
