Job Summary

The Ambulatory Population Health team is seeking a community health worker to be a part of the inpatient/outpatient Population Health program that will focus on UK Pediatric ED patients. This position will help patients navigate the healthcare system and provide support for social determinants of health. Through a risk assessment, this individual will identify those patients who need further assistance with their care and/or resources. In the outpatient setting, this position will provide information for at-risk patients regarding medical and psychosocial support services and serve as a liaison between health, social services and the community to facilitate improved health outcomes.

This role will involve building relationships with patients through trust, motivational interviewing, and empowering patients to take an active part in their health. Individuals will also provide information and resources to clients to improve their health literacy, increase retention in medical care, and improve treatment adherence to achieve better health outcomes. Primary patient interactions will occur through ED admission space, face to face, and home visits, which will aim to facilitate connection to resources, application completion, and identification of community support systems.

This position will also include community outreach to identify appropriate resources. Coordinator will work in collaboration with multidisciplinary teams to ensure patients have obtained needed resources and healthcare services and will maintain patient confidentiality and documentation in electronic health record and database.

This is a hybrid position, with both remote work and on-campus attendance expected.

Skills / Knowledge / Abilities

The ideal candidate will have excellent organizational, communication, and computer skills.

Preferred Education/Experience

High School/GED plus two or more years related experience preferred.
