Home Health Social Worker: Care Coordination Specialist

As a Medical Social Worker at CommonSpirit Health at Home, you will play a crucial role in the care coordination team. This position involves working closely with patients, physicians, and caregivers to develop personalized care plans that address the unique needs of each individual.
About the Role:
This position requires a compassionate and empathetic individual who can provide emotional support and guidance to patients and their families during challenging times. As a Medical Social Worker, you will be responsible for conducting thorough assessments, identifying social determinants of health, and connecting patients with community resources to ensure they have access to the necessary support systems.
Responsibilities:
- Conduct comprehensive assessments of patients' social, emotional, and functional needs
- Develop and implement personalized care plans that address identified needs and goals
- Provide emotional support and counseling to patients and their families
- Connect patients with community resources and services to enhance their overall well-being
- Collaborate with interdisciplinary teams to ensure seamless care coordination
Requirements:
- Masters degree from an accredited school approved by the Council on Social Work Education
- Current Social Work License/Certification through the state licensing board
- 1+ year of experience in a healthcare setting, preferably in home care
- CPR certification
