Reports To: Vice President of Clinical Services

Job Overview: The Social Work Support Specialist, as part of the clinical team, will collaborate and promote person centered strategies and recommendations, engage in case reviews, and aggregate and organize data to assist in the care management process of the individuals that we serve.

Specific Knowledge, Skills, and Abilities:

  • Knowledge of developmental disabilities, mental health, and social determinants of health.
  • Background in the school system
  • Knowledge about IEPs, CSE, and DOE resources
  • Knowledge of person-centered planning regulations.
  • Demonstrates effective leadership skills.
  • Demonstrates cultural competence.
  • Demonstrates ethical and professional responsibilities and boundaries.
  • Demonstrates capacity to use Health Information Technology to facilitate communication.
  • Knowledge of confidentiality regulations/HIPPA.
  • Ability to prioritize and complete work in timely manner.
  • Proactively approaches professional responsibilities.
  • Demonstrate a comfort level of engaging with people with behavioral health needs.

Experience:

  • Master’s Degree in the field of psychology, social work, or a related field with at least 2 years of experience working with Intellectually and/or Developmentally Disabled individuals.
  • Knowledge about OPWDD services and regulations
  • Strong computer knowledge.
  • Familiarity with person-first, person-centered planning concepts.

Essential Responsibilities:

  • Serve as part of the Clinical Team for Person Centered Services.
  • Provide case review or audit of individuals with mental health concerns.
  • Serve as a consultant in situations where prior clinical expertise is warranted.
  • Review and analyze data and trends.
  • Design, implement, and present trainings as needed.
  • Provide case review or audit of individuals with significant behavior issues and/or mental health concerns.
  • Participate in mandatory staff meetings.
  • Any other duties as deemed relevant by supervisor/organization
