Job Responsibilities throughout our Integrated Care Continuum:
· Conduct comprehensive assessments of patients' physical and mental health, substance use history, and social determinants of health.
· Develop individualized care plans based on patients' needs and treatment goals, integrating behavioral health, addiction medicine, and primary care interventions.
· Administer medications, provide medication-assisted treatment (MAT) for substance use disorders, and monitor patients' responses to treatment.
· Collaborate with an interdisciplinary team, including physicians, therapists, social workers, and other healthcare professionals, to ensure coordinated and holistic care.
· Offer evidence-based counseling and psychoeducation to patients and their families, focusing on mental health, addiction, and chronic disease management.
· Conduct regular follow-up visits to monitor patients' progress, provide ongoing support, and adjust treatment plans as needed.
· Maintain accurate and up-to-date medical records and documentation, adhering to regulatory guidelines and best practices.
· Actively participate in continuing education and professional development opportunities.
Qualifications:
· Valid nursing license (RN or LPN) in Florida
· Experience working in integrated healthcare settings, behavioral health, addiction medicine, and/or primary care
· Strong understanding of mental health disorders, addiction treatment modalities, and physical health
· Excellent assessment and critical thinking skills
· Ability to work independently and collaboratively within an interdisciplinary team
· Outstanding communication and interpersonal skills
· Compassionate, non-judgmental, and patient-centered approach to care
