As a Substance Use Case Manager with our Project In-Reach team, you'll play a pivotal role in transforming the lives of incarcerated and recently released individuals, supporting their journey to recovery and successful reentry into the community.

What You’ll Do:
Support Transition: Provide crucial pre-release (10–180 days) and post-release (120 days) services to help clients reintegrate smoothly into society.
Counseling and Case Management: Offer individualized and group counseling, crisis intervention, and case management to meet clients' unique needs.
Empower and Advocate: Assist clients in accessing mental health, housing, vocational, and employment resources, and advocate for their success every step of the way.
Collaborative Care: Work with a multidisciplinary team, law enforcement, and community partners to ensure clients receive the comprehensive support they deserve.
Skills for Success: Help clients prepare for employment through mock interviews and job linkages.
Document and Track Progress: Maintain detailed documentation of client progress, ensuring the highest standards of care.

What We’re Looking For:
Empathy and Advocacy: A heart for serving people with substance use and mental health challenges.
Knowledgeable and Resourceful: Experience in substance use assessments, motivational interviewing, and familiarity with community resources.

Organized and Detail-Oriented: Strong administrative skills to manage client documentation and follow through on service plans.
Qualifications:
Bachelor’s degree in Social Work, Psychology, or Counseling (preferred).
2+ years of experience in substance abuse/mental health services.
Certified Addiction Specialist (CADC-CAS), Licensed Advanced Alcohol and Drug Counselor (LAADC), or equivalent (preferred).
A valid California Driver’s License, CPR/First Aid Certified, and TB clearance.
