Position Summary: The Clinical Programs Specialist (CPS) organizes and oversees operations for PrimeCare Health’s specialty programs and is responsible for coordinating and supporting patient care within the clinic. Topics/areas include, but are not limited to, nutrition, yoga and/or tai chi classes, pain management, stress management, mindfulness, and smoking cessation. The CPS will focus on engaging patients through outreach, education, referrals, and marketing and will coordinate group classes that are facilitated/co-facilitated by a medical and/or behavioral health provider. The CPS supports patients in accessing specialty care, ensures appropriate transitions and continuity of care, and connects patients to community-based resources and support.
Duties and Responsibilities
- Develop and implement specialty clinical programs, including, but not limited to, weight management, nutrition, pain management, smoking cessation, and diabetes management.
- Coordinate the operations of, and organize patient attendance for, group visits/classes related to managing the impact of chronic conditions.
- Provide ongoing operational support by scheduling classes and patients, providing outreach to patients, creating outreach materials, tracking class attendance.
- Support and closely coordinate treatment with primary care and behavioral health providers who oversee specialty programs and facilitation of group visits.
- Monitor clinical outcomes through follow-up of patient visits and document patient interactions utilizing the worklist to identify and re-engage patients who may have been lost to care.
- Facilitate referrals for clinically indicated services outside the primary care clinic for linkage to social services, including, but not limited to, housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment, etc.
Required Skills or Abilities
- Demonstrated ability to collaborate and communicate effectively in a team setting and ability to maintain effective and professional relationships with patients and other members of the care team.
- Effectively engage patients utilizing telephone or face-to-face encounters.
- Strong written and verbal communication skills.
- Effective problem-solving skills and ability to analyze and use data for decision making.
- Ability to work flexible and extended hours and travel between sites, as needed.
Qualifications, Knowledge, Experience, or Licensure
- Bachelor’s degree in relevant field, with 1-3 years related experience, strongly preferred.
- Familiarity with Electronic Medical Records (EMR) system, preferably with Athena preferred.
- Experience with, or ability to learn, assessment tools and treatment planning for common mental health and/or substance use disorders, when appropriate.
- Prior experience collaborating with underserved and/or transient patients who have co-occurring mental health, substance abuse, and physical health problems.
- Bilingual in English/Spanish helpful.

Salary
All wages are based on relevant years of experience. The minimum rate is the base wage that a candidate can expect to earn with no relevant expereince to the role.
