PRC Baker Places is a non-profit residential treatment agency that has provided the San Francisco community with alternatives to institutional care since 1964. Our direct care professionals provide counseling and case management services to previously homeless adults living with co-occurring diagnoses of chronic mental illness and substance use disorder. All agency services are provided from the philosophy of social rehabilitation which integrates teaching daily living skills in a safe and healthy environment.

Position Overview

The Peer Support Specialist works as a member of a team to help build better connections and treatment follow-up for people who received services from the HIV/Hep C testing grant who are out in the community or other residential treatment settings.


Primary Duties and Responsibilities

This position involves participation and implementation of grant goals consistent with funding requirements.


Client Assessment and Engagement:

  • Conduct comprehensive assessments to determine eligibility for SSI/SSDI benefits.
  • Establishes trust, builds rapport, and maintains effective communication with members/clients to support their application process.
  • Provide outreach services to identify and engage individuals in need of assistance.
  • Obtains signed Authorization of Release of Information forms.


Social Security Application Assistance:

  • Create a "My Social Security Account" for applicants
  • Guide clients through the SSI/SSDI application process, including completing applications, gathering medical records, and submitting required documentation.
  • Ensure that applications are thorough, accurate, and submitted in a timely manner.
  • Collaborate with medical providers to obtain necessary documentation and evidence to support claims.
  • Write SOAR Medical Summary Reports for applicants.
  • Track the status of the application and assist with appeals if denied.


Peer Support:

  • Meet all HIPAA standards including but not limited to exchanging information, keeping Protected Health Information (PHI) secure, and following procedures to protect electronic transmission/access to PHI.

    • Accompany individuals to appointments at the Social Security Administration.

    • Document and maintain up-to-date information in AWARDS (Electronic Health Record) and in client files regarding services provided in compliance with state and federal regulations.
      • Maintain detailed case notes and documentation
      • Monitor the status of applications and follow up with the Social Security Administration as needed.
      • Provide continuous support to clients throughout the application process, including attending hearings and appointments if necessary.
      • Assist clients in arranging transportation.
      • Provide outreach and successful engagement to a diverse population

Collaboration and Advocacy:

    • Work closely with community organizations, healthcare providers, and social services agencies to coordinate services and resources for clients.
    • Advocate on behalf of clients to ensure they receive the support and resources they need.
    • Participate in community outreach and education efforts to raise awareness about the SOAR program.


Training and Development:

    • Participate in SOAR training sessions and remain current on best practices and changes in SSI/SSDI policies.
    • Provide training and support to other staff members and community partners on the SOAR model.


Other:

    • Adhere to organization policies and procedures, professional/ethical standards, and legal requirements regarding the SOAR process.
    • Maintains clear and healthy interpersonal boundaries, complies with laws and regulations, and adheres to all professional ethical standards, and confidentiality.
    • Other duties as assigned.

Minimum Qualifications

  • High school diploma or equivalent
  • Preferred completion of a mental health, peer counselor certificate program
  • Preferred 2 – 4 years of direct lived or supportive experience with SF behavioral health and HIV/AIDS provider systems
  • Knowledge of HIV/AIDS and Hep C, and the potential impacts to client’s life
  • Basic knowledge of Microsoft products preferred
  • Complete SOAR training within 45 days of hire
