Home Touch Healthcare, Inc. is looking for a qualified Physical Therapist with an excellent work ethic to do home health visits in the Morris and surrounding areas. We are a nurse-owned and operated organization that operates in a family-like atmosphere, and always striving to put our patients first. We are looking for a Physical Therapist with solid skills; a compassionate and caring attitude; decisions based on integrity; a service-oriented mindset; and who enjoys working in the home health setting.
 
Physical Therapist Qualifications:

•    Licensed by the state of Illinois.
•    At least 21 years of age.
•    Possess and maintains current CPR certification, or another certification process as deemed appropriate by agency, state, and federal regulations.
•    Must have reliable transportation, current driver's license, and appropriate automobile insurance.
•    Must have TB test and physical.
•    Background check will be done.

Physical Therapist Responsibilities:

When physical therapy services are provided, a physical therapist shall provide the services in accordance with the plan of treatment and within the licensee's scope of practice as established by the Illinois Physical Therapy Act.  These services shall include the following:

•    Instruct other health team personnel, including, when appropriate, home health aides and family members, in certain phases of physical therapy with which they may work with the patient.
•    Instruct the patient and family in the total physical therapy program.
•    Assist in evaluating the patient's level of function by applying diagnostic and prognostic procedures.
•    Guide the patient in the use of therapeutic creative and self-care activities for the purpose of improving function.
•    Prepare clinical and progress notes for the clinical record. Reporting on patient’s condition and significant changes to the assigned nurse or case manager.


The pay rate is $70.00 per visit. This is a PRN position. Visits are typically 2 days a week for 30 minutes to an hour per patient. This is a good position for a Physical Therapist who is currently working but could use a few extra hours.
