MINIMUM QUALIFICATIONS:

Must have a Master’s Degree in Speech-Language Pathology or similar field. Must hold a certificate of clinical competence from American Speech-Language and Hearing Association and Appropriate Speech-Language Pathology license in accordance with Illinois Department of Financial and Professional Regulations. Previous experience working with individuals with developmental/intellectual disabilities is preferred.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

The list of essential duties and responsibilities, as outlined herein, is intended to be representative of the task to be performed. The omission of an essential function does not preclude management from assigning duties not listed herein is such duties are a logical assignment to the position.

1. The Speech-Language Pathologist will collaborate with all program teams to assess and identify individuals needing additional support with functional communications/articulation deficits or swallowing difficulty.

2. In accordance with the DHS Adult Waiver, complete a speech/language assessment and work collaboratively with the assigned QIDP to seek waiver funding and provide speech services for identified individuals.

3. Develop and carry out an individualized treatment plan for identified individuals that addresses the specific functional communication needs.

4. Serve as a communication/swallowing consultant and resource for direct service staff, management and families. This could include: one on one training, smaller home meeting/trainings or larger group meetings.

5. Participate in any ISP meetings for individuals identified as needing regular communication/swallowing oversight.

6. Submit monthly progress reports for assigned individuals.

7. Consult with leadership to identify appropriate communication devices and accessories per individual/home approved for such device.

8. Set up, maintain and train on augmentative and alternative communication systems.

9. Maintain collaboration with Children’s and Adult’s program leadership and other disciplines to best serve our residents with communication and swallowing disorders.

10. Maintain session notes of all services provided.

11. Complete monthly billing (ROCS) for those individuals with DHS approved Speech Therapy funding.

12. Provide observation opportunities and/or supervision to any future Speech-Language Pathologist students.

13. Adheres to federal and state legislation, regulation and policies that affect speech and language therapy practice.

14. Maintains American Heart Association certification in First Aid and CPR, if required.

15. Successfully completes Little City Foundation required training, re-training, and any additional training as required by Little City Foundation policies and practices, or as assigned by his or her immediate supervisor.

16. Other duties as assigned.
