DEPARTMENT SUMMARY: The Senior Services department supports and enriches the lives of lesbian, gay, bisexual and transgender (LGBT) people 50 years and older, with an overarching goal of successful aging in place. In this effort, the department provides a broad array of social, educational, and support services to LGBT seniors, all of which are free or low-cost. We have an expanding population of LGBT homeless population and are growing as a department to include on site affordable housing for LGBT seniors.
JOB SUMMARY: Under the supervision of the Associate Director of Housing and Training, the Housing Coordinator is responsible for fulfilling contract requirements while providing client-focused supportive services. This role is centered on addressing housing inequities for LGBTQ+ individuals, particularly older adults, through infrastructure development, educational outreach, and community engagement activities. The Housing Coordinator will build partnerships with community organizations to create housing opportunities tailored to the senior population. The position requires extensive travel within the Los Angeles area, including working offsite in the community, visiting partnership locations, conducting home visits, and collaborating closely with the housing team to ensure a high level of teamwork.
ESSENTIAL FUNCTIONS:
• Develop and implement an outreach plan targeting insecure and/or unhoused LGBTQ+ individuals.
• Develop and maintain relationships with housing providers (e.g.: local apartment complexes, supportive housing agencies, and housing nonprofits) throughout LA County to secure stable housing and support services.
• Coordinate consistent outreach and network opportunities with community housing providers including creating annual Housing Fair to create ongoing and positive partnerships.
• Map and promote underused employment and education pathways for LGBTQ+ individuals.
• Develop, administer, and evaluate progress through the development of a tailored survey tool, partnership agreements, and a comprehensive outreach and engagement plan.
• Lead education outreach efforts to assist housing-insecure and/or unhoused LGBTQ+ older adults in securing permanent housing.
• Remove barriers and leverage resources to support permanency within established communities.
• Organize and deliver Pathways to Housing and Housing Readiness and other supportive workshops in response to community needs.
• Develop personalized housing plans, provide documentation support, and assist with housing placement forms and lottery participation.
• Foster collaborations with landlords and homelessness services providers by offering move-in assistance including subsidy acquisition.
• Monitor service provision and housing placements using the CIVI internal tracking database.
• Initiate community engagement activities to support housing retention and relational permanency.
• Plan and conduct a kick-off event to foster connections among participants.
• In collaboration with local agencies and/or partnerships, provide financial literacy training and host regular community events for resource sharing and support.
• Offer comprehensive case management services and eviction prevention workshops in collaboration with legal services.
• Encourage participant involvement in the Advisory Council and enroll them in skills and vocational training.
• Track participation, service delivery, and housing retention throughout the grant period using the CIVI database.
• Administer surveys at designated intervals post-housing acquisition to gather feedback on support, community integration, and retention.
• Evaluate program success, aiming for a 70% positive response rate regarding perceived support and sustainability.
• Implement continuous improvement processes based on community feedback.
• Represent the Center at community meetings regarding housing issues for homeless populations and assist with educating the community on changing policy as it relates to LGBTQ+ seniors’ access to affordable housing.
• Document and track the progress of program objectives as outlined in the grant, ensuring accurate and timely reporting.
• Assist with the preparation and submission of grant reports by compiling data on outreach efforts, housing placements, community partnerships, and program outcomes.
• Collaborate with the team to monitor key performance indicators and ensure alignment with grant requirements, contributing to the overall success of the program.
• Ensure new client data compliance is completed (Civi registration, photo, ID Card, move in date, etc.).
• Provide linkage to resources such as DPSS, Social Security, Medi-Cal, CalFresh, and act as liaison for clients with those resources.
• Match clients with appropriate housing opportunities through field-based housing search, including but not limited to short-term crisis housing, and affordable housing.
• Work with clients and their current and previous supports to develop individual case plans to help them meet initial and intermediate goals in the areas of education and employment, housing stability, financial stability, positive healthy outcomes, and permanent connections.
• Maintain statistical client data, including entering client data into HMIS Clarity database, completing standardized screening, and entering point of contact.
• Connect with participants and landlords when they are available (this may include evenings and weekends.)
• Coordinate move-in assistance.
• Other duties as assigned.
JOB QUALIFICATIONS AND EXPERIENCE:
• Knowledge of or experience working with the LGBTQ community and familiarity with issues of particular relevance to LGBTQ people.
• A passion for the Center’s work and its mission to make the world a better place for LGBTQ people.
• Bachelor’s degree in social work, Public Administration, or a related field.
• 2-3 years’ experience providing housing case management with increasing responsibilities.
• Strong understanding of the challenges faced by LGBTQ+ individuals in securing stable housing.
• 2+ years’ experience providing direct services to unhoused community members with substance use, mental health, and trauma histories who require compassion, patience, and strengths-based models of service provision.
• 2+ years’ experience in developing and delivering educational workshops.
• Proven ability to develop and maintain partnerships with community organizations, landlords, and other stakeholders.
• Experience in delivering educational workshops and providing case management services.
• Excellent organizational, communication, and leadership skills.
• Proficiency in data tracking and evaluation tools, such as CIVI or similar platforms.
• AHA CPR certification.
• Flexibility to work evenings and weekends as needed.
• Intermediate to advanced knowledge of Microsoft Excel, Word, and PowerPoint.
• Bilingual (English/Spanish) skills a plus
• Valid CA driver license in good standing with access to reliable transportation with car insurance.
• Demonstrated ability to work effectively with people of diverse races, ethnicities, nationalities, sexual orientations, gender identities, gender expression, socio-economic backgrounds, religions, ages, English-speaking abilities, immigration status, and physical abilities in a multicultural environment.
