Enhanced Multi-Disciplinary Team Coordinator

(E-MDTC)

POSITION SUMMARY:

Enhanced Multidisciplinary Teams (E-MDTs) are county-based teams that bring together various disciplines to intervene in cases of older adult abuse, including financial exploitation, physical abuse, psychological abuse, sexual abuse, and neglect by others. E-MDTs review cases of elder abuse and multidisciplinary approaches are used, as appropriate, to address issues identified. Team members include local professionals from various disciplines, including but not limited to Adult Protective Services, aging services, health and human services, and law enforcement to provide an effective and efficient means of addressing cases of abuse of older adults (aged 60 and older). The enhancement comes with access to forensic accountants, mental health professionals, and civil legal services.

In this role, the E-MDTC will oversee the CNY E-MDT Hub. They will be responsible providing program leadership for a minimum of 2-4 counties, scheduling meetings, preparing and maintaining agendas, and compiling pertinent case information. This position will participate in the Elder Justice Committee; attend Partner Meetings and support and assist the Enhanced Multi-Disciplinary Team Program Coordinator in related projects.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Represent the Agency to the community and collaborate with community members to ensure the implementation and/or maintenance of the Agency’s programs and services as well as organize outreach and program campaigns.
  • Maintain regular communication and foster positive relations among MDT members.
  • Collect, analyze and report program outputs and outcome data.
  • Collaborate with leadership and other staff to bring the necessary resources, training and professional development to community service partners.
  • Collaborate with staff to bring the necessary resources and training for staff development.
  • Ensure the Agency’s services are maintained in accordance with the Agency’s mission and goals, maintain the database consistent with Vera House practices.
  • Manage difficult and/or emotional client situations and respond promptly to client needs.
  • Ensure a warm and welcoming environment to all visitors.

EDUCATION, SKILLS & EXPERIENCE:

  • Bachelor’s degree in human services related field required. Master’s degree preferred.
  • One to three years’ experience in direct service and program management preferred.
  • Demonstrated experience in training, writing and public speaking also preferred.
  • Thorough knowledge of elder abuse as well as domestic and sexual violence, experience working with older adults is beneficial.
  • Ability to develop a positive, compassionate rapport with survivors of domestic violence, sexual assault or other violent trauma.
  • Strong proficiency with Microsoft Office Suite.
  • Knowledge of community-based service delivery systems, in particular through knowledge of criminal and civil justice systems.
  • Strong planning, organizational, and time management skills.
  • Excellent interpersonal, oral communication and written skills, with the ability to solve complex problems and demonstrate critical thinking.
  • Adaptability to work independently, or collaboratively with the team, and leadership.
  • Must possess a valid New York State Driver’s License and have access to a reliable vehicle for travel throughout the area.
  • Maintain strong professional boundaries, limited to the extent of professional relationships, ensuring compliance, ethical and appropriate interactions.
  • Completion of a satisfactory background check.

SALARY RANGE

$20-$23 per hour (non-exempt)
