Job Description:

The social worker is a member of an interdisciplinary team responsible for meeting the psychosocial needs of the patient. Responsibilities may include case management, advocacy, and planning services. The social worker may also facilitate the development of a discharge plan and may coordinate after care continuity. The social worker provides education to staff, patients, and families as needed. Daily duties may include but are not limited to:

  • Provide patient/family psychosocial assessment upon admission; ongoing family systems assessment as needed
  • Demonstrates a working knowledge of current theory/practice; integrates such information into practice
  • Completes all documentation according to discipline/unit standards
  • Participates in the development/implementation of an interdisciplinary plan of care
  • Provides supportive counseling to patients/families according to treatment plan
  • Assists in the discharge process in collaboration with the treatment team, patient, and family
  • Develop/maintain an understanding of the culture, traditions, values, beliefs that relate to health care and to decision making

Qualifications:
  • MSW from an accredited graduate school of social work.
  • Current licensure by the State of Connecticut as a Licensed Master Social Worker.
  • Licensed Master Social Worker (LMSW) must achieve (LCSW licensure) within two years of date of hire.
  • Competence in meeting the needs of patients admitted to the hospital for treatment of complex medical issues, chronic disease and/or rehabilitation regardless of age or disability as determined through the interview process, previous work experience, and education.
  • Required: One year clinical experience in a medical setting.
Job Type:

This is a contract position with a duration of 3 Month(s) cover a leave of absence. Successful contracts lead to additional employment opportunities.

Work Site:

This is a fully on-site position in New Britain, Connecticut.
