Job Summary

Clinical Social Worker sought for a contract position at Actalent. The successful candidate will be a member of an interdisciplinary team responsible for meeting the psychosocial needs of patients. Key responsibilities include case management, advocacy, and planning services. The social worker will also facilitate the development of a discharge plan and coordinate after-care continuity. Additionally, the social worker will provide education to staff, patients, and families as needed.

Responsibilities
  • Conduct patient/family psychosocial assessments upon admission and ongoing family systems assessments as needed
  • Stay current with theory and practice, integrating knowledge into daily work
  • Complete documentation according to discipline/unit standards
  • Participate in the development and implementation of an interdisciplinary plan of care
  • Provide supportive counseling to patients and families according to treatment plans
  • Assist in the discharge process in collaboration with the treatment team, patient, and family
Qualifications
  • Master of Social Work (MSW) from an accredited graduate school of social work
  • Current licensure as a Licensed Master Social Worker (LMSW) in the State of Connecticut
  • Licensed Master Social Worker (LMSW) must achieve Licensed Clinical Social Worker (LCSW) licensure within two years of hire
  • Competence in meeting the needs of patients with complex medical issues, chronic disease, and/or rehabilitation
  • One year of clinical experience in a medical setting
