Position: Care Manager Registered Nurse
Location: UNC Medical Center at Chapel Hill - 101 Manning Dr. Chapel Hill, NC 27514
Shift: M-F, weekend and holiday rotation.
Responsibilities: The Care Manager RN plays a crucial role in providing comprehensive and coordinated care to patients within UNC Health. This position involves utilizing a variety of skills, including risk segmentation, patient assessments, patent-centered care plans, tasks or interventions, care transitions, delegated work, and payer communication.
Pay: Starting at $34.65 and increases with experience plus benefits
Must Haves:
· North Carolina or Compact RN license
· BLS certification
· 2-years acute care experience (Preferred experience includes Oncology and Medicine)
