**Job Summary:**

We are looking for an experienced LPN/LVN who is passionate about providing exceptional care and service to our patients and families. **Responsibilities:**

The successful candidate will be responsible for:

  • Providing high-quality, patient-centered care that meets the unique needs and preferences of each individual.
  • Conducting thorough assessments, developing and implementing comprehensive care plans, and documenting patient progress and outcomes.
  • Maintaining accurate and up-to-date medical records, ensuring compliance with regulatory requirements and Company policies.
  • Communicating effectively with patients, families, and healthcare professionals to ensure seamless care coordination and collaboration.
  • Participating in ongoing education and professional development to stay current with best practices, technologies, and regulatory updates.

**Requirements:**

To be considered for this role, applicants must:

  • Hold a current, unrestricted state license as a Licensed Practical Nurse (LPN/LVN).
  • Have a minimum of six months of recent experience as a Licensed Nurse in a clinical care setting.
  • Be able to travel to patient locations, which may involve exposure to varying environments and conditions.
  • Possess excellent time management, organizational, and problem-solving skills, with a focus on prioritizing tasks and managing multiple responsibilities.

**Preferred Qualifications:**

  • Home health experience.
  • Demonstrated proficiency in electronic medical records (EMRs) and other relevant software applications.

Aveanna Healthcare offers a competitive salary range of $60,000 - $80,000 per year, depending on experience and location, as well as:
