Community Crisis Stabilization Nurse

The Community Crisis Stabilization (CCS) Licensed Practical Nurse (LPN) plays a vital role in delivering medical assistance, care coordination, and rehabilitation services to individuals experiencing crises, facilitating their stabilization and reintegration into the community. The LPN is tasked with providing direct nursing care to address the health maintenance needs of identified individuals. This position operates within a multidisciplinary team framework to support clients at the CCS across various aspects of their lives and to aid in achieving their personal goals.
The CCS LPN is responsible for:
- Creating a trauma-informed atmosphere that fosters safety and recovery.
- Educating clients and staff on topics such as medication management, vital signs monitoring (pulse, temperature, blood pressure, blood glucose), and participating in medication administration, documentation, and shift change reporting in line with program protocols.
- Contributing to psychiatric, substance use, and nursing evaluations, as well as risk management, safety planning, discharge planning, and care coordination.
- Supervising individuals in a crisis stabilization environment under the guidance of the Nurse Manager and the direction of psychiatric nurse practitioners and psychiatrists.
- Collaborating with clients and their support systems to implement person-centered interventions from their treatment plans aimed at crisis stabilization and swift community reintegration, while documenting client progress throughout their stay at the CCS.
- Advocating for clients and coordinating care with other healthcare providers and care coordination entities, ensuring smooth transitions for continuity of care.
- Maintaining accurate documentation and records.
- Providing transportation for appointments and facilitating community integration efforts.
- Reporting any changes in clients' conditions or unusual occurrences to the Nurse Manager or Clinician On-Call promptly; engaging in collaborative problem-solving to support individual clients and the therapeutic environment.
- Participating in assigned training sessions and maintaining all necessary certifications; actively engaging in supervision, staff meetings, family meetings, and discharge planning activities.
- Ensuring that all individuals receiving treatment at the CCS are treated with dignity and respect, adhering to the program's Human Rights policies.
- Executing all responsibilities in accordance with agency policies and procedures.
- Performing additional duties as assigned.

Qualifications
- A high school diploma or GED is required; completion of an accredited nursing program is mandatory.
- A minimum of one (1) year of experience as an LPN; experience with individuals diagnosed with mental health conditions in community settings is preferred.

- Awareness of social determinants of health and commitment to crisis intervention within a holistic care framework.
- A valid driver's license is required at the time of employment. Candidates must be at least 21 years old, have held a valid US driver's license for at least one year, and have access to an insured, reliable vehicle.
- Successful completion of a multistate CORI, SORI, reference check, employment verification, education, and credentialing background check is required.
