Job Summary

We are seeking a skilled and compassionate Nurse Care Specialist to join our team. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and support to our residents. Responsibilities

  • Deliver high-quality patient care in accordance with facility clinical systems, regulatory requirements, and physician orders.
  • Oversee the direction of Nursing Assistants to ensure the highest quality services are delivered in a resident-centered environment.
  • Assess residents' needs and assist in developing and following individual care plans.
  • Identify and communicate changes in residents' status to physicians, NP, staff, and family members.
  • Administer medications, treatments, and other direct care as prescribed and indicated.
  • Coordinate admissions, transfers, and discharges of residents.
  • Communicate information to staff to provide a smooth shift transition.
  • Document appropriately as needed or required in PCC.

Requirements

  • Current and unrestricted IL LPN/RN License
  • CPR certification
  • Excellent verbal and non-verbal communication skills
  • Skilled nursing facility experience preferred
