Job Title: MDS Coordinator

Liberty Healthcare & Rehabilitation Services is seeking an experienced MDS Coordinator to join our team.

Job Summary:

We are looking for a skilled and detail-oriented MDS Coordinator to maintain and follow a schedule of due dates for all MDS, coordinate the completion of the MDS by all disciplines, and review the MDS data for accuracy.

Responsibilities:

  • Maintain and follow a schedule of due dates for all MDS.
  • Coordinate the completion of the MDS by all disciplines.
  • Review the MDS data for accuracy and meet with appropriate staff as needed to assure accuracy.
  • Complete the MDS and input into the computer.
  • Verify that assessments have been transmitted and approved by the State in a timely manner.
  • Correct any rejected records and prepare them for re-submission.
  • Complete CAAs according to state guidelines.
  • Formulate Resident Care Plan (RCP) along with interdisciplinary care plan team for all residents in accordance with their needs and within the required time frame.
  • Schedule and conduct RCP meetings on a regular and timely basis.
  • Communicate the RCP to appropriate staff members and notify supervisor if noncompliance by staff is noted.
  • Work with all in-house and ancillary departments to assure understanding and compliance with the RCP.

Requirements:

  • Registered Nurse, graduated from an accredited School of Nursing and have a current, valid RN license from the North Carolina Board of Nursing and submit proof of license renewal every other year.
  • Ability to read, know, and follow personnel, department and facility policies and procedures and adhere to local state and federal requirements.
  • Experience with MDS-RAP and Care Planning functions.
  • Prefer experience with RUG-IV.
  • Prefer experience with MDS 3.0.
