Job Summary

We are seeking an experienced and skilled MDS Coordinator to join our team at Liberty Healthcare & Rehabilitation Services. As a key member of our clinical operations team, you will be responsible for ensuring the accuracy and timeliness of Minimum Data Set (MDS) assessments and care plans for our residents.

Key Responsibilities
  • MDS Management: Maintain and follow a schedule of due dates for all MDS assessments, ensuring timely completion and accuracy.
  • Interdisciplinary Collaboration: Coordinate the completion of MDS assessments by all disciplines, including nursing, therapy, and other healthcare professionals.
  • Data Review and Verification: Review MDS data for accuracy and meet with staff as needed to ensure accuracy and completeness.
  • Assessment and Care Planning: Complete MDS assessments and input data into the computer system, ensuring accurate and timely transmission to the state.
  • Corrective Action: Correct any rejected records and prepare them for re-submission, ensuring compliance with state regulations.
  • Care Plan Development: Formulate Resident Care Plans (RCPs) in collaboration with the interdisciplinary care team, ensuring resident needs are met and care plans are developed within required timeframes.
  • Communication and Education: Communicate RCPs to staff members and notify supervisors of non-compliance, ensuring understanding and adherence to care plans.
  • Departmental Collaboration: Work with in-house and ancillary departments to ensure understanding and compliance with RCPs.
Requirements
  • Education and Licensure: Registered Nurse (RN) degree from an accredited school of nursing and current, valid RN license from the North Carolina Board of Nursing.
  • Experience and Skills: Experience with MDS-RAP and Care Planning functions, with preference for experience with RUG-IV and MDS 3.0.
