We are seeking a highly skilled and dedicated Registered Nurse to fill the position of MDS Coordinator at our 90-bed skilled nursing facility in Charlotte. As an MDS Coordinator, you will play a critical role in ensuring the highest quality of care for our patients by completing minimum data set assessments and developing comprehensive plans of care.
Key Responsibilities- Conduct thorough and accurate minimum data set assessments to identify patient needs and develop individualized care plans.
- Collaborate with interdisciplinary teams, including physicians, nurses, and therapists, to ensure seamless care coordination.
- Develop and implement care plans that meet the unique needs of each patient, taking into account their medical history, functional abilities, and personal preferences.
- Ensure compliance with federal and state regulations, as well as company policies and procedures, related to MDS assessments and care planning.
- Communicate effectively with patients, families, and healthcare teams to ensure that care plans are met and that patients receive the highest quality of care.
- RN license and current registration in good standing.
- Minimum 2 years of experience in a skilled nursing and rehabilitation environment.
- Strong clinical skills, including assessment, planning, and implementation of care.
- Knowledge of the MDS 3.0 process and ability to apply it in a clinical setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to work independently and as part of a team, with a strong commitment to service excellence and customer satisfaction.
