Job Overview

Core Responsibilities:

The Residential Independence Facilitator reports directly to the Assistant Director of Residential & Individualized Services and operates in accordance with the policies set forth by the Board of Directors. This role is pivotal in the creation and execution of our independent living program, providing essential support for individuals in supportive housing, In Home Supports (IHS), and safety net services. The facilitator will play a crucial role in implementing enabling technology and overseeing the designated Supportive Housing and IHS programs, ensuring that services align with the latest trends and best practices aimed at empowering individuals with disabilities.

Key Duties:

  • Develop and implement an independent living curriculum to assist candidates in achieving greater independence.
  • Guide and support staff in planning, coordinating, and delivering comprehensive services that promote optimal growth and care for individuals.
  • Assist in the recruitment and supervision of direct support professionals in Supported Housing, safety net, and IHS services.
  • Provide oversight and support to individuals in supportive housing and IHS, including conducting regular team meetings and individual planning sessions.
  • Collaborate with support staff and individuals to enhance knowledge of enabling technology and digital literacy, fostering independence.
  • Ensure the provision of independent living support, training, and education to staff and individuals, promoting a safe and responsible environment.
  • Maintain effective communication with the Assistant Director regarding program information, issues, and recommendations.
  • Work closely with the Assistant Director to manage financial oversight and billing for the Supportive Housing program.
  • Stay informed about quality service regulations established by DDS and ensure compliance with CCARC's quality assurance standards.
  • Exhibit strong interpersonal skills to enhance communication between individuals and staff.
  • Be available for 24-hour on-call administrative support for emergencies related to supportive housing.
  • Conduct regular site visits to supportive housing locations to ensure consumer satisfaction.

Additional Responsibilities:

  • Work collaboratively as a team member and remain adaptable.
  • Stay updated on current trends and methods in alignment with CCARC policies.
  • Organize and participate in various team meetings, including individual, residential staff, clinical, and interdisciplinary meetings.
  • Maintain open communication with participants, families, staff, and relevant stakeholders.
  • Obtain certification for conducting assistive technology assessments.

Technology Proficiencies:

  • Proficient in Microsoft Office Suite (Word, Excel, Outlook) and comfortable using tablets and smartphones.
  • Ability to work with databases and learn new technical skills.
  • Familiarity with assistive and enabling technology to enhance individual independence.
  • Experience with THERAP and Sandata EVV systems is preferred.

Qualifications:

  • Bachelor's Degree in a related field with a minimum of 2 years of experience in developmental disabilities and supervisory roles, or a High School/GED with at least 4 years of relevant experience.
  • Completion of agency-required certification and training.
  • Valid Driver's License and insurability with the agency's carrier, along with access to a vehicle as needed.
  • Ability to follow both oral and written instructions.

Physical Requirements:

  • Must possess the physical and emotional health necessary for effective job performance.
  • Ability to perform lifting and transferring tasks and manage potential risks associated with challenging behaviors.
