Responsibilities

  • Assesses patients' needs and provides personalized guidance and support via phone.

  • Provides education and support to patients and families regarding health conditions, treatment options, and self-care strategies.
  • Prioritizes and manages multiple calls simultaneously, focusing on urgent situations and ensuring timely follow-up care.
  • Collaborates with interdisciplinary teams to coordinate care and achieve optimal outcomes.
  • Participates in quality improvement initiatives to enhance patient satisfaction and outcomes.
  • Stays current with industry developments, best practices, and regulatory requirements.
