Position Overview:
The Case Manager will be responsible for providing investigative and comprehensive case management services for a caseload involving child and adult abuse or neglect. This role requires immediate and follow-up actions for clients at risk, ensuring they receive the necessary protective services and support.
- Conduct timely, face-to-face assessments of reported child maltreatment cases to identify and address safety concerns.
- Develop and implement safety plans to address identified risks promptly.
- Engage in diligent search efforts to locate families when initial contact is unsuccessful.
- Utilize Solution Based Casework (SBC) methodologies in case management.
- Collaborate with children and parents to create and utilize the Family Functioning Agreement, Action Plan, and Family Plan aimed at achieving desired outcomes.
- Complete genograms to map out the family support network.
- Work with community partners to establish systems of support for families and children.
- Ensure adequate frequency and quality of visits with families to promote safety, permanency, and well-being.
- Identify and address the physical, educational, and mental health needs of children and families.
- Set permanency goals for children in care.
- Ensure stability in foster care placements and living situations that align with the best interests of the children.
- Make concerted efforts to achieve timely permanency for children in accordance with relevant regulations.
- Facilitate sibling placements in foster care unless separation is necessary for individual needs.
- Maintain connections for children with their community, family, and friends.
- Support positive relationships between children in care and their biological parents by encouraging meaningful interactions.
- Document all case management activities in compliance with established policies.
- Celebrate and acknowledge positive outcomes with families.
- Utilize a personal vehicle for work-related travel, including transporting children as needed.
- Participate in after-hours on-call duties as required.
- Engage in ongoing professional development and learning opportunities.
- Attend and provide testimony in court hearings as necessary.
Compensation: $33.50 per hour
Mileage Reimbursement: $0.625 per mile
