Care Coordinator Job Summary

CMP is seeking a skilled Registered Nurse to serve as a Care Coordinator for our Hospice Care team. As a key member of our interdisciplinary team, you will play a vital role in ensuring the highest quality of care for our patients and their families.

Key Responsibilities:
  • Assess patient needs and develop individualized care plans in collaboration with the healthcare team.
  • Coordinate and facilitate communication among healthcare providers, patients, and families to ensure seamless care transitions.
  • Provide education and support to patients and families on disease management, treatment options, and end-of-life care.
  • Collaborate with the interdisciplinary team to identify and address patient needs, including pain management, symptom control, and spiritual support.
  • Document patient information, progress, and outcomes in accordance with regulatory requirements and CMP policies.
Requirements:
  • Current CA State licensure as a Registered Nurse.
  • Current CPR certification.
  • One year experience as a professional nurse.
  • Valid CA drivers license and an automobile, with current registration and insurance, for field work.
Compensation:

$44.38 - $72.40 per hour
