Hospice RN Care Coordinator Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our Hospice team as a Care Coordinator. In this role, you will work closely with our interdisciplinary team to provide high-quality patient care and support to individuals and their families.

Key Responsibilities:
  • Assess patient needs and develop individualized care plans in collaboration with the healthcare team.
  • Coordinate and facilitate patient care activities, including medication management, wound care, and symptom management.
  • Provide emotional support and education to patients and their families, promoting a holistic approach to care.
  • Collaborate with healthcare providers to ensure seamless transitions of care and optimal patient outcomes.
  • Document patient information and progress in a timely and accurate manner.
Requirements:
  • Current CA State licensure as a Registered Nurse.
  • Current CPR certification.
  • One year experience as a professional nurse.
  • Valid CA drivers license and an automobile, with current registration and insurance, for field work.
Compensation Information:

$44.38 / hourly - $72.40 / hourly
