Care Coordinator Specialist

We are seeking a highly skilled Care Coordinator Specialist to join our team at Choice Of New Rochelle In. The successful candidate will provide comprehensive case management services to assigned clients, including crisis intervention, advocacy, and referrals.
Key Responsibilities:
- Assess client needs and develop care plans to achieve self-sufficiency
- Conduct field work, including home visits and escorting clients to appointments
- Execute care activities in accordance with client goals and health home acuity rating
- Maintain and submit necessary documentation and reports
- Develop and maintain relationships with community services and resources
Requirements:
- Bachelor's degree required
- Valid driver's license and clean driving record required
- Strong interpersonal skills and ability to work as part of a team
- Strong computer skills and knowledge of Microsoft Office
