Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Physician Network team at Northeast Georgia Health System. As an LPN in this role, you will be responsible for providing high-quality patient care and collaborating with physicians to ensure the best possible outcomes.

Key Responsibilities
  • Assess and Plan Care: Conduct initial assessments of patients and develop plans of care in collaboration with physicians.
  • Implement and Evaluate Care: Implement nursing interventions and evaluate patient progress, making adjustments to care plans as needed.
  • Communicate Effectively: Communicate patient information and care plans to physicians and other healthcare team members through written documentation and verbal reports.
  • Medication Administration: Correctly administer and document medication regimens for patients.
  • Infection Control and Safety: Identify and implement infection control and safety practices to ensure a safe environment for patients and staff.
  • Patient Care Coordination: Organize patient care and prioritize tasks to ensure timely implementation of physician orders.
  • Education and Training: Serve as a clinical resource for medical office assistants and other non-licensed staff, and conduct in-services to educate center staff on clinically related topics.
Requirements
  • Reading, Writing, Reasoning, Talking, Fingering: Possess strong reading, writing, reasoning, and communication skills, as well as the ability to perform fine motor tasks.
