Purpose and Scope

The Social Worker position at Little City Foundation is responsible for delivering social work services to students with special needs in accordance with Illinois State Board of Education requirements.

Key Responsibilities
  1. Develop and implement IEP goals based on formal and informal assessment results that align with social emotional learning standards.
  2. Participate in IEP meetings and 3-year re-evaluations as required throughout the school year.
  3. Submit quarterly progress reports and ESY reports to lead teachers and team members in a timely manner.
  4. Track data on a weekly basis to update quarterly benchmarks related to IEP goals.
  5. Collaborate with teams to develop and implement interventions for students not making expected progress.
  6. Provide and document direct and consult therapy minutes as required on each student IEP.
  7. Consult with and collaborate with school teams regarding recommendations throughout the school year.
  8. Participate in rotating classroom clinical meetings and bi-weekly clinical meetings as needed.
  9. Complete therapy notes and minute logs, as well as enter specific data points into the goal tracking system.
  10. Establish and maintain consistent communication with teachers, related service team, case managers, and parents/guardians as needed.
  11. Provide support, resources, and recommendations for classrooms as needed.
  12. Work with team members to model strategies, discuss student progress, and assess student needs.
  13. Manage inventory of therapeutic equipment and assessments, and project needs for budget planning.
  14. Comply with professional code of ethics and school policies, and adhere to federal and state legislation, regulation, and policies that affect social work therapy practice.
  15. Maintain American Heart Association certification in First Aid and CPR, as well as National Crisis Institutes CPI certification, and complete any other required trainings.
Qualifications

The ideal candidate will have a Master's in Social Work, Professional Educator License with an endorsement in School Social Work, and Licensed Clinical Social Worker certification. Previous experience working with students with special needs/autism is preferred.

Physical Requirements and Work Environment

The employee will be subject to sitting, walking, and standing for prolonged periods, and will frequently grasp, lift, hold, or feel objects. The employee will occasionally stoop, kneel, crouch, or crawl, and will be required to use manual and finger dexterity and eye-hand coordination when working with persons served and handling office, medical, or household equipment. The employee will be subject to assist and lift up to 30 pounds with varying amounts of assistance on a reoccurring basis, and will be required to have corrected vision and hearing within normal range and the ability to operate a motor vehicle. The employee will be required to perform CPR and CPI, and will be exposed to blood, body fluids, infectious substances, including communicable diseases when performing personal care (Category III). The employee will occasionally be exposed to cleaning agents and disinfectants, and will work in a school environment with a noise level of normal to loud.
