The Pharmaceutical Care Assistant plays a crucial role in delivering medication management and direct support to residents, ensuring a nurturing and supportive environment. Key Responsibilities:
1. Oversee the daily functions of the medication area and medication delivery as instructed by the Health Services Director (HSD), adhering to physician directives and regulatory standards.
2. Process and manage physician medication and treatment requests in compliance with state laws and nursing regulations.
3. Administer and facilitate the self-administration of both routine and specialized medications as per physician instructions and regulatory guidelines.
4. Execute basic treatments and monitor residents as directed by the HSD, in line with medical orders and state regulations.
5. Maintain precise documentation of medication and treatment services provided or missed, including controlled substances, in each resident's health record.
6. Coordinate with pharmacies for medication orders and refills.
7. Report any medication discrepancies according to established protocols.
8. Document and communicate changes in resident health status, incidents, and accidents to the HSD, physician, and family members as required, completing all necessary incident reports.
9. Regularly liaise with physicians, pharmacies, external services, residents, and families regarding medication and care services.
10. Provide direct care to residents while promoting their dignity, independence, and choices, and consistently observe and report changes in their physical and mental health to the HSD or relevant directors.
11. Assist with or deliver care services as outlined in the resident's personalized service plan.
12. Welcome residents, visitors, and staff courteously, ensuring respect for dignity and confidentiality while promoting individual rights.
13. Adhere to community fire safety and infection control protocols, promptly reporting any unsafe conditions or equipment to the supervisor.
14. Serve meals in the dining area, assist residents during mealtime, manage meal tray delivery, and help maintain cleanliness in the dining and kitchen areas.
15. Aid in lifting, moving, and transporting residents using appropriate techniques or assistive devices as per the resident's service plan.
16. Support tours, transitions, and coordination of personal care services with colleagues, responding promptly to assistance requests.
17. Administer First Aid and initiate emergency procedures in accordance with policies and state regulations.
18. Ensure proper storage of medications and equipment, maintaining a clean and organized medication area.
19. Dispose of outdated or discontinued medications in accordance with pharmacy policies.
20. Participate in training, in-services, and team meetings.
21. Perform additional duties as assigned.

Qualifications:
Preferred experience includes one or more years of personal care with older adults or as mandated by state regulations. Must possess state-required medication administration certification and complete all necessary training and competency assessments. Proficiency in word processing is required. Current CPR certification, First Aid training, and Food Handlers permit are also necessary as per state regulations. Completion of all state-mandated training is essential.
Education:
A high school diploma or GED, or equivalent knowledge and skills acquired through a combination of education, training, and experience.
