Job SummaryWe are seeking a compassionate and skilled Registered Nurse (RN) to join our team at Amedisys. As a Hospice Nurse, you will provide high-quality patient care and support to patients and their families in the comfort of their own homes.
Responsibilities- Perform initial nursing assessments on new admissions
- Develop, prepare, and maintain individualized patient care progress records with accuracy and timeliness
- Regularly communicate patient progress to the clinical manager, physician, and care team
- Educate families on patient condition, needs, and care
- Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so
- Perform on-call responsibilities and on-call services to patients/families as assigned
- Current RN license, specific to the state(s) you are assigned to work
- One year of nursing experience preferred
- Current CPR certification
- Valid driver's license, reliable transportation, and liability insurance
