Blue Ridge Hospice is seeking a compassionate and skilled Registered Nurse to join our team. As a Registered Nurse, you will provide high-quality care to patients and their families in the comfort of their own homes.
Job Summary:The Registered Nurse will provide visits to patients with emergent and critical needs, working independently to cover patient needs from 4:30 pm to 8:30 am Monday-Friday, and from 4:30 pm Friday until 8:30 am Monday. The ideal candidate will have experience in nursing, critical thinking skills, and the ability to work effectively in a fast-paced environment.
Responsibilities:- Provide professional nursing care in line with the established patient care goals, ensuring all actions are aligned with the physicians plan of care.
- Assess and evaluate patient status, reporting findings to the RN on duty.
- Develop and update care plans that establish goals based on nursing diagnoses and incorporate palliative nursing actions.
- Counsel patients and families on nursing and related needs, providing health care instructions as per the assessment plan.
- Teach self-care techniques to patients and caregivers, including medication, diet, and health counseling opportunities.
- RN license in the Commonwealth of Virginia.
- Graduated from an accredited nursing school.
- BSN preferred.
- 2 years of experience in hospice, home health, or ICU/CCU, preferred.
- CHPN certification, preferred.
- Basic knowledge of Medicare/Medicaid reimbursement regulations.
- Current CPR Certification.
- Strong communication and interpersonal skills.
- Proficient computer skills and electronic medical records required.
