As an RN Care Manager at Strive, you will play a crucial role in delivering care to patients with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD). Your responsibilities will include:
Collaborating with the care team to develop individualized care plans that address both clinical and non-clinical patient needs. Conducting regular assessments, both virtually and in-person, to identify any medical or social issues that may arise. Proactively reaching out to patients and clinics to assess their status and provide necessary support. Managing a diverse patient panel while adhering to NCQA standards for complex case management. Communicating effectively with care teams and family members to ensure understanding of health statuses and care plans. Acting as a liaison with dialysis facilities to facilitate smooth transitions in care. Utilizing data analytics to generate reports that identify trends and areas for improvement. Providing culturally competent care that meets the diverse needs of our patients. Applying clinical judgment and analytical skills to ensure high-quality care delivery.Qualifications
Associate's degree in nursing from an accredited institution. Valid Registered Nurse (RN) license in the state of practice. Minimum of 3 years of relevant experience or education. Current BLS or CPR certification is required. Reliable transportation for patient home visits. Ability to provide in-person care, which may involve physical activities. Adequate internet connectivity for remote work.Preferred Qualifications
3+ years of experience in nephrology, emergency, intensive care, or complex case management nursing. Certification as a Case Manager (CCM) is preferred. Proficiency in Microsoft 365 Suite for a hybrid work environment.About You
