Role Overview

The RN Care Manager plays a pivotal role in delivering patient care for those with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD). Acting as a crucial resource for patients, families, and healthcare teams, the RN Care Manager is responsible for coordinating services to enhance care delivery. While primarily conducted through telecommunication, this position may require on-site visits to healthcare facilities or patient residences as needed.

Key Responsibilities

  • Collaborate with the care team to develop personalized care plans that address both clinical and non-clinical patient needs.
  • Conduct regular assessments, both virtually and in-person, to identify any medical or social issues and environmental changes that may pose risks.
  • Proactively reach out to patients and clinics to assess their status, provide assistance, and address any inquiries.
  • Manage a diverse panel of patients, evaluating their needs while adhering to NCQA standards for complex case management.
  • Communicate updates to the care team and family members, ensuring patients understand their health status and providing tailored education.
  • Act as a liaison with dialysis facilities, ensuring smooth transitions in care and maintaining accurate records.
  • Analyze reports using Strive data platforms to identify trends and areas needing attention.
  • Deliver culturally sensitive care that meets the diverse needs of Strive patients, promoting collaboration among clinical team members.
  • Utilize clinical judgment and analytical skills to provide high-quality care consistently.

Minimum Qualifications

  • Associate's degree in nursing from an accredited institution.
  • Active Registered Nurse (RN) license in the relevant state.
  • Minimum of 3 years of relevant education, experience, or certification.
  • Current BLS or CPR certification.
  • Reliable transportation for commuting to patient locations.
  • Ability to provide in-person care, which may involve physical activities such as standing, walking, and lifting.
  • Internet connectivity with specified minimum speeds.

Preferred Qualifications

  • Over 3 years of experience in nephrology, emergency room, intensive care, or complex case management nursing.
  • Certified Case Manager (CCM) certification is preferred.
  • Proficiency in Microsoft 365 Suite and electronic medical records.

  • Proven ability to build strong relationships with patients and families to promote engagement and optimal outcomes.
  • Experience in proactively contacting patients to assess their needs.
  • Familiarity with documenting and managing care plans through electronic medical records.
