We are seeking a compassionate and skilled Registered Nurse to join our Afterhours Hospice team in Corpus Christi, TX. As a key member of our team, you will provide end-of-life care to patients and their families outside of regular business hours.
Responsibilities- Conduct comprehensive and timely delivery of hospice care for patients and their families after regular business hours.
- Triage patient needs over the phone until a nurse can provide an in-person visit.
- Respond to crisis calls from patients and families afterhours with urgency and empathy.
- Make initial nursing evaluations to determine eligibility for hospice services.
- Perform afterhours visits, including admission visits, death visits, recertification visits, and routine visits.
- Identify and address patient/family physical, psychosocial, emotional, and environmental needs.
- Document patient/family problems, goals, interventions, and response to hospice care.
- One year of clinical experience.
- Current RN licensure in the state of practice and one year of clinical experience.
- Current CPR Certification.
- Current driver's license, valid vehicle insurance, and access to a dependable vehicle.
