Job Summary

The Registered Nurse - Hospice Care Specialist plans, organizes, and directs hospice care, with a focus on community health education and experience. This role involves building from the resources of the community to plan and direct services that meet the needs of individuals and families within their homes and communities.

Key Responsibilities

Patient Care
  • Conduct comprehensive assessments of patients and families to determine hospice needs.
  • Provide physical assessments and histories of current and previous illnesses.
  • Deliver professional nursing care by utilizing the nursing process.
  • Assess and evaluate patient status by writing and initiating plans of care, regularly re-evaluating patient and family/caregiver needs.
  • Participate in revising plans of care as necessary.
  • Use health assessment data to determine nursing diagnoses.
  • Develop care plans that establish goals based on nursing diagnoses and incorporate palliative nursing actions.
  • Initiate preventive and rehabilitative nursing procedures.
  • Administer medications and treatments as prescribed by the physician.
  • Counsel patients and families in meeting nursing and related needs.
  • Provide health care instructions to patients as appropriate per assessment and plan.
  • Assist patients with activities of daily living and facilitate their efforts toward self-sufficiency and optional comfort care.
  • Act as Case Manager and assume responsibility to coordinate patient care for assigned caseloads.
Communication
  • Complete, maintain, and submit accurate and relevant clinical notes regarding patient conditions and care given.
  • Record pain/symptom management changes/outcomes as appropriate.
  • Communicate with physicians regarding patient needs and report changes in patient conditions; obtain/receive physicians' orders as required.
  • Communicate with community health-related persons to coordinate care plans.
  • Teach patients and families/caregivers self-care techniques as appropriate.
  • Work in concert with the interdisciplinary group.
  • Provide and maintain a safe environment for patients.
  • Assist patients and families/caregivers and other team members in providing continuity of care.
  • Work in cooperation with families/caregivers and hospice Interdisciplinary Group Members to meet the emotional needs of patients and families/caregivers.
  • Attend interdisciplinary group meetings.
Additional Responsibilities
  • Participate in on-call duties as defined by the on-call policy.
  • Ability to work weekends.
  • Ensure that arrangements for equipment and other necessary items and services are available.
  • Supervise ancillary personnel and delegate responsibilities when required.
  • Fulfill the obligation of requested and/or accepted case assignments.
  • Actively participate in quality assessment performance improvement teams and activities.
Job Requirements
  • Graduate of a National League for Nursing accredited school of nursing.
  • Minimum of two years of experience, at least one of which is in the area of public health, home care, or hospice nursing.
  • Registered nurse with current licensure to practice professional nursing in the state.
  • Demonstrates excellent observation, problem-solving, verbal, and written communications; nursing skills per competency checklist.
  • Shows ability to organize and prioritize workload independently.
  • Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, and/or crouching. Visual acuity and hearing to perform required nursing skills.
  • Maintains a current CPR certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
  • Self-directed and able to work with minimal supervision.
