Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team at HealthCare Talent. As a Level of Care (LOC) RN, you will be responsible for conducting home visits, assessing patient needs, and developing care plans to ensure the best possible outcomes for our geriatric patients.

Key Responsibilities
  • Conduct initial and annual Level of Care assessments and send to the State for approval
  • Record participant assessments, reassessments, and care delivered in the medical record
  • Review necessary documentation and coordinate with other disciplines for bi-annual LOC audit preparation
  • Remain attentive to participants' health status and social functioning, and assist with identification of overt problems/needs
  • Develop a Plan of Care based on nursing process that incorporates immediate and continuing care needs
  • Provide direct care as assigned, including wound care, IV therapy, blood sugar checks, and vital sign checks
  • Consult with and educate participants, family members, and caregivers regarding disease processes, self-care techniques, and medical interventions
  • Ensure that all tests, lab work, and diagnostic studies ordered by the Primary Care Physician have been carried out
  • Coordinate all medication for BCSC, including stocking non-prescription medications and administering medication in the PACE Center
Requirements
  • Fluent bilingual Spanish/English skills are required
  • Graduate of a school of professional nursing (ASN, BSN, MSN)
  • Current License as RN through California Board of Registered Nursing in good standing
  • Verification of completion of CPR (BLS) and first aid training
  • Valid California driver's license and reliable vehicle
  • At least one year of experience as an RN, preferably in home health or hospice working with the frail or elderly
  • Excellent computer skills (MS Office) and strong communication skills
