Job Title: Case Management Specialist

We are seeking a highly skilled and compassionate Case Management Specialist to join our team. As a Case Management Specialist, you will be responsible for providing comprehensive case management services to children and families in need.

Key Responsibilities:
  • Develop and implement individualized case plans to meet the unique needs of each child and family.
  • Provide crisis intervention and support to families in crisis.
  • Collaborate with other service providers and professionals to ensure comprehensive support for children and families.
  • Document progress and maintain accurate records in accordance with agency policies and procedures.
  • Participate in weekly case staffing and team meetings to discuss case progress and challenges.
Requirements:
  • Bachelor's degree in social work, psychology, or a related field.
  • Three years of experience in case management or a related field.
  • Ability to communicate effectively in writing and verbally in English and Spanish.
  • Ability to work independently and as part of a multidisciplinary team.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
