Home Health Registered Nurse Job Opportunity

Montefiore Medical Center is seeking a skilled Registered Nurse to join our Home Health team in Englewood Cliffs, New Jersey.


Job Summary:
We are looking for a dedicated and compassionate Registered Nurse to provide high-quality patient care in the comfort of their own homes. As a Home Health Registered Nurse, you will be responsible for assessing patients' needs, developing and implementing individualized care plans, and collaborating with healthcare teams to ensure seamless care transitions.

Responsibilities:
• Conduct thorough patient assessments to identify needs and develop effective care plans
• Provide direct patient care, including medication management, wound care, and education
• Collaborate with healthcare teams to ensure comprehensive care and coordinate services
• Educate patients and families on disease management, medication adherence, and self-care techniques
• Document patient progress and outcomes in a timely and accurate manner

Requirements:
• Current RN licensure in New York State
• 2+ years of experience in Home Health or a related field
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Knowledge of Medicare conditions of participation and home health regulations
