Home Health Care Registered Nurse Job Opportunity

Montefiore Medical Center is seeking a skilled Registered Nurse to join our Home Health team in Englewood Cliffs, New Jersey.


Job Summary:
We are looking for a dedicated and compassionate Registered Nurse to provide high-quality patient care in the comfort of their own homes. As a Home Health Care Registered Nurse, you will be responsible for assessing patients' needs, developing individualized care plans, and delivering evidence-based care to ensure optimal health outcomes.
Responsibilities:
  • Conduct thorough assessments of patients' physical and emotional needs
  • Develop and implement individualized care plans to meet patients' unique needs
  • Provide skilled nursing care, including wound care, medication management, and education
  • Collaborate with interdisciplinary teams to ensure comprehensive care
  • Document patient progress and outcomes

Requirements:
  • Current RN licensure in New York State
  • 2 years of experience in Home Health care
  • Ability to educate and motivate staff
  • Knowledge of Medicare conditions of participation
