Job Title: Home Health Care Registered Nurse

We are seeking a skilled and compassionate Registered Nurse to join our Home Health Care team at Montefiore Medical Center. As a Home Health Care Registered Nurse, you will play a vital role in providing high-quality patient care and support in the comfort of patients' homes.

About the Role:
  • Provide comprehensive nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
  • Collaborate with patients, families, and healthcare teams to develop and implement individualized care plans.
  • Conduct regular home visits to assess patients' needs and provide education on self-care and disease management.
  • Work closely with other healthcare professionals to ensure seamless care coordination and communication.
