Job Description

We're seeking skilled and compassionate Home Health Physical Therapists to join our team. As a Home Health PT, you'll have the opportunity to work with patients who have lost mobility, strength, and functionality due to an illness or injury.

You'll be part of a multidisciplinary home health team that includes an RN, LPN, CNA, HHA, OT, and SLP, all working together to create personalized plans of care for each patient.

Your responsibilities will include observing movements, conducting assessments, documenting progress, and modifying care plans as needed.

  • Assist patients with exercises and hands-on therapy to manage pain and increase mobility.
  • Educate patients on their plan of care, established goals, exercises, and self-care.
  • Coach family members on the plan of care, therapy goals, and safely assisting with exercises.
Required Skills and Qualifications

To succeed in this role, you'll need:

  • A graduate degree from an accredited Physical Therapist program.
  • An active PT license in SC.
  • Knowledge of state and federal home health regulations.
  • Good clinical judgment, strong interpersonal skills, dexterity, and compassion.
